The Process
Step 1: Prior to the Intake Interview
You must fax or email a copy of the following documents:
- Court order
- Current photograph of child(ren)
- Driver’s license
- Car insurance - if transporting the child(ren)
Step 2: Schedule an Intake Interview
The interview must be conducted in-person with each parent individually before your first visit. During the interview, we will go over the following:
- Complete the Agreement for Services
- Complete the Emergency Medical Release (Custodial Parent)
- Complete the Do’s and Don’ts Form
- Review the fees and method of payment
- Set up a visitation schedule based upon your court order
Step 3: During the Visits
A written record for each case will be maintained, including, but not limited to the following:
- The date, time and duration of the visit
- How the visit begins and ends
- Each activity performed
- All parent-child contact
- Any reminders of the guidelines from the monitor to the non-custodial parent
- Whether or not the visit is terminated and details of the termination
- Dropoff/Pickup details by both parents
The Provider
I am a trained Professional Supervised Child Visitation Provider offering both off-site supervised monitored visitations and "exchange-only" services under California Rules of Court, Standard 5.20. I comply with the California Rules of Court 5.20 and Family Code 3200.5.
As a non-partisan observer, I work with Family Law attorneys, social workers and individuals to faithfully carry out the stipulations of the court order relating to the days, hours and times of the visitations. I will closely observe the behavior and interactions between the non-custodial parent and his or her child(ren). My goal is to maintain a safe, secure and comfortable environment so that all parties can enjoy the child-parent visitations.
I am college educated and listed in the registry of Trustline, a State of California agency that provides background checks for child providers. I have also successfully passed multiple background and fingerprints checks in CA (Los Angeles County DCFS) and NV (Registered Private Investigator). I am certified in adult/child/infant CPR/First Aid.
I serve the Conejo Valley/Ventura County areas including Agoura, Calabasas, Camarillo, Moorpark, Simi Valley, Thousand Oaks, Westlake Village and surrounding areas.
